I certify that I have properly familiarized myself with the description of the mountain ascent or hike and the safety rules. I am aware of various risks, including the possibility of sustaining injuries and/or bites that may be life-threatening, and I have received comprehensive answers to all my questions.
I clearly understand the rules presented to me and the importance of following them. I also realize that ignoring the rules can lead to injuries, fractures, and accidents during the route.
I also confirm the absence of any medical contraindications for my participation in hikes and mountaineering. I assess the child’s health status (congenital, acquired, chronic, acute diseases and/or disorders, as well as any deviations from the norm) independently and/or based on my doctor’s advice, and I make the decision to proceed with the proposed routes myself, taking the health status into account.
I have been properly informed that the hike guide/organizer bears no responsibility for health issues and/or deterioration of health resulting from failure to follow rules and instructions.
I have been properly informed that the organizer and staff bear no responsibility for the possible loss, damage, and/or total destruction of personal property during the hikes.
